Job Summary

Reporting to the Director of Social Services/Social Services Supervisor, the Case Manager provides and documents clinical and case management services for assigned clients. The Case Manager will be an active listener and provide high quality customer service and respond by exhibiting cross-cultural awareness with the ability to promote and contribute to an equitable and inclusive anti-racist organizational culture and environment.

Key Responsibilities & Essential Function

Program/Facilities

1. Provides comprehensive case management services for assigned clients including, but not limited to:
2. Support and resources/referrals for clients to meet objectives, including but not limited to:
3. Entitlements
4. Housing
5. Employment / Education / Vocational Training
6. Health, mental health treatment, and substance abuse services
7. Social and community integration
8. Counseling and support to clients on an on-going basis including, but not limited to:
9. Crisis intervention
10. Training and support on client daily living skills, as required
11. Assistance in problem solving, as required
12. Conflict resolution
13. Advocacy
14. Assesses client needs, develops service plans and/or service referrals to achieve client self-sufficiency.
15. Conducts field work including, but not limited to home visits, escorting clients to appointments, and outreach.
16. Facilitates client groups.
17. Monitors client medications, as required.
18. Conducts client intake and updates information in client database system(s).
19. Completes incident reports and distributes related notices within 24 hours.

Administrative

1. Maintains and submits all necessary documentation and reports accurately in a timely manner and as required by Urban Pathways, regulatory and funding agency policies, and procedures including but not limited to:
2. Enter progress notes after each client meeting within 48 hours and/or as mandated by the Program.
3. Completes Intake/Admission documentation within 24 hours of client admission, as mandated by the Program.
4. Documents assessments in client records within required timeframe (not applicable for Outreach Program).
5. Updates information in client records, as required.
6. Documents incidents and distributes related notices within 24 hours when the Case Manager has witnessed the incident.
7. Submit all required paperwork for supervisor approval within 48 hours.
8. Files paperwork in client charts (once approved/signed by supervisor), as required by program (no more than 48 hours).

Community Relations

1. Develops and maintains relationships with community services and resources with particular emphasis on substance/alcohol abuse programs, housing, health and mental health programs, employment, and benefits.
2. Attends and participates in external meetings with funding agency and housing providers.

Communication

1. Communicates verbally and in writing in a professional manner at all times.
2. Attends staff meetings, trainings, and supervisory sessions, as required.
3. Responds to all communications including telephone, email, and other oral and written inquiries in a timely manner.

Performs all other duties as assigned and as required.

Qualifications, Education and Experience

• Bachelor’s degree required. Bachelor’s degree in social work or other related field preferred.
• Minimum one year of case management, individual or group counseling experience preferred.
• Knowledge of NYC housing subsidy programs.
• Experience with people experiencing homeless, mental illness and/or substance abuse population preferred.

Skills & Competencies

• Proficiency in Microsoft Office (e.g. Word, Excel, Outlook, etc.) and electronic record-keeping systems (e.g. Awards, Cares, Caires, etc.).
• Ability to maintain accurate and up-to-date case notes, documentation, and records.
• Excellent organizational and time management skills.
• Excellent communication, interpersonal abilities, and follow-up skills.
• Work effectively with individuals from diverse backgrounds and cultures.
• Foreign languages a plus.

Other

• Required to work a flexible schedule including weekends, holidays, and evenings.

Capabilities

• Desire to perform the essential functions of this job.

• Comfortable with using public transportation to travel to and from various programs sites.

Licenses

• Valid driver’s license with clean driving record a plus.

Physical Requirements

• All programs require fieldwork, home visits, client escorts, and use of public transportation.
